Weight-loss medicines called GLP-1 receptor agonists were first developed for diabetes and obesity, but researchers are now asking a fascinating question: could they also help treat addiction? The idea is not as strange as it sounds. These drugs do more than reduce appetite and slow digestion. Scientists think GLP-1 signals also act in the brain’s reward system, which helps control craving and the urge to keep seeking something pleasurable. That is why alcohol, nicotine and even opioids have become part of the discussion around these medicines. (nature.com)

The biggest recent clue came from a BMJ study published on March 4, 2026. Researchers followed 606,434 US veterans with type 2 diabetes for up to three years. Compared with people who started SGLT-2 inhibitors, those who started GLP-1 drugs had lower risks of new alcohol-, cannabis-, cocaine-, nicotine- and opioid-use disorders. Among veterans who already had a substance use disorder, GLP-1 treatment was also linked to fewer addiction-related emergency visits and hospital admissions, a lower risk of overdose, and about half the risk of addiction-related death. Still, this was an observational study, so it shows an association, not definite proof that the drugs caused these benefits. (pubmed.ncbi.nlm.nih.gov)

Clinical trials give a more careful picture. In a 2025 randomized trial of 48 adults with alcohol use disorder, low-dose semaglutide reduced the amount of alcohol people drank in a laboratory test, lowered drinks per drinking day, and reduced weekly alcohol craving. In a subgroup of smokers, it also reduced cigarettes per day. But an earlier trial of exenatide in 127 patients did not significantly reduce heavy drinking days overall, even though brain scans suggested weaker responses to alcohol cues in reward-related brain areas. (pubmed.ncbi.nlm.nih.gov)

So, can obesity drugs treat addiction? The most honest answer is: maybe, but it is too early to say yes with confidence. The evidence is exciting, especially because current addiction treatments are limited, but larger and longer trials are still needed before GLP-1 drugs become standard treatment for addiction.
